You have made a typo or error when entering your first or last name:

If we already have your identity document, you can send us your request using this contact form.

Otherwise, please send us an up-to-date identity document or birth certificate via this same form.

If you have changed your first or last name in the case of a change of marital status or sex :

Send us your request, including your up-to-date ID, to this contact form.

The family record book, marriage certificate and divorce judgment are also accepted as proof of marriage/divorce.
